Height And Weight In Children – How can parents react to a child gaining weight in a pandemic without making things worse? Image author: Seyyed Llata

Nadine Sabbah’s 10-year-old daughter has always loved being physically active. Before the pandemic, she did four hours of gymnastics a week and, like most kids her age, spent hours running around with friends in her community. But then came COVID-19. Her gym classes were canceled and she stayed home all day instead of exercising regularly.

With the news about the virus, which was so widespread and uncertain at the beginning of the pandemic, the girl was afraid to even walk around her village.

But when Nadine’s mother noticed that her daughter was self-conscious about her body, she began to worry. “She started gaining weight and it really affected her behavior,” says the Lebanese/French expat, who also has a 6-year-old daughter. “She started getting anxious and we noticed she was only wearing baggy clothes to hide her body.”

Nadine is far from the only one worried about her baby’s weight due to the pandemic. According to research published in JAMA Network Open, lockdowns have shrunk our worlds—closing schools, gyms, and travel options—and made us all bigger.

According to a March 2021 study of 7,500 people, people gain an average of 1.8 pounds per month while ordering shelter in the US. And while data on obesity in children is currently scarce, experts believe that the same trend can be seen in the pediatric population in the developed world.

“We can safely assume that there has been an increase in the number of children gaining weight during the pandemic,” says Jordana Smith, a dietician at Genesis Clinic in Dubai Healthcare City. “In my practice, I’ve certainly seen an increase in overweight children and parents who are concerned about their children’s weight.”

With group sports and classes canceled or limited and extra caution around going out, children’s physical activity levels dropped, while screen time and access to sugary snacks skyrocketed.

“Children and teenagers go to sleep later and wake up later, which disrupts their ‘normal’ schedules,” says Dr. Nancy Ahmed Radian Ghanem, a pediatric specialist at Prime Medical Center in Dubai.

“The impact of late nights is growing every day and affects other aspects of children’s lives – such as meals, nutrition and physical activity.”

Online schooling eliminates the possibility of children going to class, participating in physical education classes or running during recess, all of which provide great exercise that you wouldn’t notice on a normal day, says Dr. Add to that the stress of life in a time of uncertainty, children can respond by reaching for sugary and fatty snacks, which leads to unhealthy eating habits that can contribute to weight gain and jeopardize their long-term health, Nancy adds.

At a time when so many things seemed out of control, food was one area where both children and adults could exercise some form of autonomy, says Nadia Brooker, consultant psychologist at Priory Wellbeing Center Dubai – who has also witnessed the rise of eating disorders, anorexia among her patients. “The current COVID-19 pandemic is having a significant impact on the daily lives of everyone – young and old,” says Brooker.

“There’s so much uncertainty and change going on right now that it’s really hard to track and manage your physical and emotional well-being.

We all experience many, difficult emotions such as stress, sadness, fear, boredom and loneliness, which can often lead to changes in appetite that can eventually lead to disordered eating patterns such as binge eating. – Nadia Brooker, consultant psychologist at The Priory Dubai

Especially in children and adolescents, increasing screen time goes hand in hand with a sedentary lifestyle, so lack of movement also facilitates weight gain.

Unlike adults, children should gain weight evenly as they grow. But when does childhood weight gain slide from healthy to unhealthy? “Typical weight gain depends on the child’s age, but the range would be between 1.8-3kg in weight per year and 5-7cm in height until the child reaches puberty,” says Genesis Clinic dietitian Jordana Smith.

During puberty, childhood growth rates double, explains Prime Medical Center’s Dr. Nancy, “but since some children start developing as early as age 8 and some as early as age 14, it can be normal for two children to of the same sex, height and age have very different weights.”

He says that children between the ages of 11 and 14 gain weight on average between 13.5 and 18 kg. “A child at this age can gain 9 kg or more in a year.”

For children, we also look at their ‘percentile’ – which is the percentage where your child falls in height and weight compared to other children – and their BMI (body mass index). “We need to have 3% of babies and children that fall into the 3rd and 97th percentiles, as these are common population standards,” explains Jordana Smith. “We don’t all look the same; we have small or short people and we have taller people – this is where genetics comes into play.”

However, we cannot tell whether a child is overweight or underweight just by looking at their centiles. “You should look at their curve (at least 3 points) and then compare their weight and height charts (or BMI charts) to see where they are,” says Jordana, stressing that we also need to remember that BMI ranges are different for children than for adults.

A bit of rapid weight gain here is not necessarily a cause for concern for children – it can be normal for them to grow for a while before gaining height. But Jordana Smith says a parent should be concerned about a child’s weight (under or over) when centiles cross or equalize on their growth charts. “This should be a sample for three consecutive measurements. It’s not just one point, we have to look at the sample and take into account the genetics of the family.

“If your child is above the 100th percentile, I wouldn’t automatically panic, but I would start looking at the entire history of their growth chart and definitely look at genetics and their height.”

As a general rule, a BMI of 95 or higher indicates obesity, says Dr. Nancy, while percentiles between 85 and 95 indicate a child is overweight, “so parents need to be aware of this and monitor their children carefully for any sudden weight gain that could indicate any underlying problems or even gradual abnormal weight gain, which can lead to obesity.”

Is there anything cuter than a chubby baby or toddler? “It may be sweet, but it’s not always healthy,” says Jordana Smith, a dietitian at the Genesis Clinic in Dubai.

“It all comes back to looking at their charts and seeing their growth over time. I will also take into account whether the child was breastfed or formula fed. In general, breastfed babies are expected to be smaller due to the composition of breast milk than formula-fed babies, who are expected to be larger due to the higher protein content of formula.

“‘Dog fat’ is the fat that kids have when they’re younger, but it starts to disappear as the child moves into adolescence and starts to grow. I’m usually not too concerned about a little puppy fat – I’d still educate the whole family about better food choices and exercise.

“If the rollers are on the arms, legs and back, that might ring an alarm bell, but if they’re in the middle and on the face, I wouldn’t worry too much.”

Child Growth Chart

It is well known that childhood overweight or obesity has a significant impact on physical and psychological health. Children are more likely to remain obese into adulthood and develop diseases such as diabetes, cardiovascular disease and asthma. The emotional impact of being overweight is also well documented, with children more likely to be teased or bullied because of their size, which can lead to anxiety and depression.

Many parents think that a heavy child is fat and believe that their overweight or obese child will simply outgrow it, but the opposite is true, says Dr. Nancy. “In a 2014 study published in the New England Journal of Medicine, researchers assessed changes in obesity levels in more than 7,000 children between kindergarten and high school. The results showed that obesity in kindergarten increased the likelihood of obesity in high school.

Dr. Nancy also points out that obesity increases the risk of a child contracting the coronavirus:
